    I have received this sample previously. After entering my information and clicking submit, it came up and told me only one sample per household.

    Really?? It went through for me again!! Weird.. maybe try again? Also, here's another tip. Say You live on a street called "Example". When filling out your address information, put "Example St."

    Maybe that will help it go through?
