    It took three tries, but I finally was able to complete the form and received a confirmation. It says it take 8-10 weeks. I check in with you when I receive my sample.

    like mentioned below, can’t get past request sample, until second try, then got to first question, then can’t go any further. oh well, guess i will try again tomarrow.

    If your sample isn’t working, check the URL link. If there is a # sign after the last backslash on the link, delete the # sign and hit refresh. The sample should work then. Hope this helps!
